MBLEx Massage Therapy Exam — Everything You Need to Know in 2026

The MBLEx (Massage & Bodywork Licensing Examination) is administered by the Federation of State Massage Therapy Boards (FSMTB) and is required for licensure in 46+ states. It has 100 questions and a 110-minute time limit.

MBLEx uses pass/fail scoring — FSMTB does not report a numeric percentage to candidates, though the passing threshold is approximately 70%. The seven domains are: Client Assessment, Reassessment and Treatment Planning (17%), Ethics, Boundaries, Laws and Regulations (16%), Benefits and Effects of Soft Tissue Manipulation (15%), Guidelines for Professional Practice (15%), Pathology, Contraindications and Special Populations (14%), Anatomy and Physiology (12%), and Kinesiology (11%).

Client Assessment and Ethics together account for about a third of the exam. These are the areas where candidates most often fall short. Understanding contraindications, appropriate draping, scope of practice, and documentation requirements is critical.

You need 70% to pass — that's at least 70 correct out of 100 scored questions.

Register at fsmtb.org. The exam fee is $265. After FSMTB approves your application, schedule your Pearson VUE appointment. Testing centers are available nationwide.

You must complete your state's required massage therapy education program before applying. Requirements vary by state — some states have their own license application process separate from FSMTB registration.

The Bureau of Labor Statistics puts the median massage therapist salary near $48,000, but ranges vary widely. Many massage therapists work part-time or on a per-session basis. Per-session rates range from $60 to $150+ for a 60-minute session depending on the market and setting. Medical massage in clinical settings (orthopedic, sports medicine, cancer care) typically pays more than general spa work and often includes benefits.
